The Family Home Provider Program (FHPP) offers individuals with developmental disabilities a setting to meet their unique needs while allowing as much independence as possible.

Family Home Providers are well-trained and compassionate people who embrace sharing their lives with individuals in their homes. Day Spring staff works closely to support the provider and the individual utilizing a person-centered approach. The team works together to ensure the individual’s health, wellbeing, and success.

Providers assist with daily living, medical needs, and transportation to appointments and outings in the community. One-on-one support promotes an ongoing personal bond and relationship. The FHP provides moral support, advice, and encourages self-advocacy. The Program Manager supports the provider by lending support and ensuring compliance.

If you are interested in receiving residential services through the Family Home Provider Program or in serving as a Family Home Provider please contact the FHPP Director at (502) 636-5990 or send an email HERE.

Our residential options include small group homes, staffed residences, apartment buildings, in-home services, and family home providers.

Our programs are person-centered and include 24/7 staff residential support for daily living, community access services, transportation assistance, financial management, and community living supports.
