The Family Home Provider Program

The Family Home Provider Program (FHP) offers a setting for people with disabilities to meet their unique needs while allowing as much independence as possible.

Family Home Providers are well-trained and compassionate people who embrace sharing their lives with individuals in their homes. The team works together to ensure the individual’s health, wellbeing, and success. Not only are residents embraced by their family, they are given opportunities that will cater to the person’s needs and interests.

​Providers assist with daily living, medical needs, and transportation to appointments and outings in the community. One-on-one support promotes an ongoing personal bond and relationship. The FHP provides moral support, advice, and encourages self-advocacy. Constant communication is made with the Family Home Provider Program Manager to ensure support and compliance.
